Request an appointment.

A GP referral is required before we can schedule a full psychiatry appointment. The process is structured so we can confirm suitability, keep you safe, and make the most of your time with the psychiatrist.

Getting an assessment appointment

A GP referral is required before a full assessment can be scheduled.

Make an appointment with your regular GP and explain that you would like to be assessed — for ADHD, anxiety, depression, burnout, or another mental health concern — and that you would like a referral to SetMind for the assessment.

Your GP can send the referral to us using our secure online form, by email, or by fax. If your GP has given you a signed copy, you can also email it to us directly at hello@setmind.com.au.

SetMind will email or SMS you within 5 working days with instructions, detailed costs, and some online forms to complete to begin the process.

What your GP should include

  • Your reason for referral and current concerns
  • Relevant medical history, including medications
  • Recent blood pressure, pulse, and any relevant blood tests or ECG
  • Medical clearance information where relevant
  • Risk and safety information if applicable

See your GP

Make an appointment with your regular GP and explain that you would like to be assessed for ADHD, depression, anxiety, or another mental health concern, and that you would like a referral to SetMind.

Your GP sends the referral

Your GP can submit our secure online referral form, or send the referral by email or fax. If your GP has given you a signed copy, you can also email it to us.

We review your referral

SetMind reviews the referral to confirm suitability, safety, and that all required information is included. We will contact you or your GP if anything else is needed.

We contact you with next steps

Within 5 working days we will email or SMS you with instructions, detailed fees, and online forms to begin the process.

Forms & questionnaires

What you will be asked to complete.

You will be asked to complete a short pre-assessment form and submit it. We will then send you a patient intake form, lifestyle form, and service agreement to complete and submit. Depending on your referral, you may also receive the following questionnaires.

We know that paperwork can feel hard, especially if you are worried about ADHD or have been putting things off. To book an appointment, we need every form completed — even if some questions do not seem relevant. If you need help, email us and we will call you back.

  • A short pre-assessment form
  • Patient intake form

  • Autism screening questionnaire (where relevant)
  • Mood assessment questionnaire (where requested)
  • Diagnostic Interview for ADHD in Adults (DIVA-5) — one copy for you and one for a relative or person who knew you aged 6 to 12 years (for ADHD assessments)

Your online assessment appointment

What to expect on the day.

Appointments are held by secure video call from any quiet, private location — a tablet, iPad, or computer works well. You will need a stable internet connection and a working camera and microphone.

Detailed instructions on how to join your appointment will be emailed to you. You will also receive an email reminder seven days before your appointment, followed by an SMS reminder two days before.

Your initial assessment appointment will take about 45 minutes. Please join the waiting room a few minutes before your scheduled time. If you are not in the waiting room when the appointment starts, your psychiatrist will call you from a "no caller ID" number — please answer.

After your online assessment appointment

Next steps and treatment planning.

After your assessment is completed, a comprehensive report will be sent to your GP for discussion. We suggest booking an appointment with your GP about 14 days after your SetMind appointment so your GP can discuss the findings and recommendations with you.

Before we can make a treatment appointment, your GP may need to complete some medical tests. These are only required when stimulant medication is being considered, and they help identify any health concerns that could affect the safety of those medications.

If medication is recommended and your GP is not authorised to initiate Schedule 8 medication, your GP will need to refer you back to SetMind for a medication review. This is a Medicare requirement. If your GP is authorised to prescribe, they will advise you when you need a follow-up medication review with SetMind.

If medication is not recommended, your GP will advise you on the next steps and non-medication options.
